Documentation of Disability

Students are responsible for obtaining and submitting all required documentation to the Office of Accessibility Services (OAS). Documentation should be clear, comprehensive, and current, and must be submitted with sufficient time to allow for review and the arrangement of any approved accommodations.

Acceptable Documentation

Documentation may include:

  • IEPs, 504 Plans
  • Recent psychological or psychoeducational evaluations
  • Letters or reports from licensed healthcare providers

Provider documentation must:

  • Be on official letterhead with name, license number, contact information, and signature
  • Include a clear diagnosis and date of diagnosis
  • Explain how the disability impacts current functioning
  • Support the need for specific accommodations

Please note: Previously approved or recommended accommodations do not guarantee the same accommodations will be approved by OAS. Documentation is kept confidential and not released without written permission from the student.
